The fourth moth puzzle can be found in a vast room packed with skeletons of Inferius. To make the platform you are standing on reach all the way to the ceiling, put a moth in the key on the west side of the room, stand in the gap to the west of the key, then cast Depulso three times on the key.

To reach the door, leave the platform by the northern exit, turn the key three more times, and then rapidly cast Lumo to bring the moth up here. When you get back down to the basement, take the moth from the northeast corner and place it on the pedestal in the southwest corner of the chamber.

To get the moth from the pedestal to the door, you need to return to the elevator and turn the key to the southwest position. The third moth can be found in a nook just beside the entrance. The moth puzzles are now all solved.
